The survey performs best with Internet Explorer or FireFox 23
In return for the customers time they will be entered into a drawing to win $5000 cash or 1 Million PC Plus points
Employees of Loblaw and Ipsos are banned from entering the sweepstakes
The approximate retail value of the prize draws is $6,000

Besides taking the Store Opinion survey online customers can also dial 1-877-234-2322 to take the survey and still be entered into the 5k drawing. Please note in order to be entered into the PC Plus Points drawing the customer must be a registered PC Plus Member prior to taking the survey. The customer needs to use the exact email address when taking the survey as the one used when signing up for the PC Plus program (failure to do so will result in the customer only entering the 5K Store Opinion drawing).

Rules associated with the Loblaw Customer Survey Sweepstakes?

There will be a total of twelve (12) Contest Segments throughout the promotion
The survey will end on 1/4/16 BUT may be the sponsor (aka Loblaw) reserves the right to extend the survey if they deem it required
Winners will be notified by email or phone within 4 business days of the drawing
The draw will be done by computer and will be random
Two “back up winners” will be drawn
The back up winners will be used in the event the initial Loblaw Customer Survey Sweepstakes winner fails to claim their prize

On a 3×5 index/note card be sure to include the contestants name, address, phone number and the words “Loblaws Inc. Customer Satisfaction Survey. The Store Opinion Survey will be administered by Ipsos-Reid Corporation who are based at 160 Bloor Street East, Suite 300, Toronto, Ontario M4W 1B9.
